By registering your child in the "Ignite" program you agree to receive communications from the "Ignite" leaders for the purposes of pastoral care of your child and/or the organisation of activities. Please indicate the means by which you wish to receive these communications below.

You may also wish to give permission for the leaders to contact your child, outside of the regular group meetings, directly. If you decide to give that permission, please indicate the method by which your child should receive these communications.

Please also note that:

1. The purpose of any contact or communication with your child will be for pastoral care (encouraging your child in the Christian faith, asking for prayer points and to check in on their wellbeing); and/or for organising program activities(to invite them to group meetings/events and to confirm attendance).

2. All GPC Youth leaders are required to abide by the Breaking the Silence Code of Conduct and will be taking the following measures when and if they contact your child:

• A minimum of 2 leaders able to observe the communication at all times
• Female leaders will contact females, and male leaders will contact males
• No active communication will take place after 10PM or before 6AM
• Leaders will follow the same safe ministry practices that are expected in person - accountability, transparency and exercising wisdom.

I understand that no medications will be administered to my child during the program unless I specifically request and authorise such administration. I understand that it is my responsibility to provide any such medication.

By signing this form, I authorise the leaders of this program, in the event of an emergency, to obtain at my expense any medical, ambulance or similar services considered necessary. 

I give permission for the following persons other than myself to pick up my child from this program, as per a personal arrangement: 

I accept that any unacceptable behaviour on the part of my child may result in my child being temporarily or permanently prohibited from attending this program.

I confirm that the information contained on this Registration Form is true and correct. 
I agree that I will ensure that any changes to this information are advised in writing as soon as possible. 

Any personal and sensitive information collected will be used only for purposes relating to the spiritual, pastoral, social, educational, administrative, legal and historical functions of the Church in accordance with the PCNSW‘s Privacy Policy in accordance with the Privacy Amendment (Private Sector) Act 2000. Your acceptance of this written advice will be regarded as your consent to collect and so use the information as described. If you do not consent please advise the GosfordPC Youth leaders immediately. A copy of the PCNSW‘s Privacy Policy is available on request from the GPC Session Clerk. Personal information will not be used for any other purpose without first obtaining your consent.

The Children’s Ministry leadership team is bound by the Code of Conduct of the “Breaking the Silence” procedures of the Presbyterian Church of NSW.
